• Report a Missing Person

    Please read the following carefully before filling out the form. Do NOT fill out the form if you do not meet the requirements listed:

    • Eligible Cases: Open cases of missing children or adults which have been reported to a U.S. law enforcement agency and are entered into the NCIC as a missing person can be submitted.

    • Who is eligible to fill out this form: Immediate family members, which include spouse, biological parent(s), legal guardian(s), adult sibling(s), or adult children of the missing person may submit this form.

    • Permissions being granted to Project Jason: Submitting this form grants permission to Project Jason to disseminate photos and information on the submitted missing person in an attempt to locate him/her, using either electronic means or other forms of communication.

    • Responsibilities of Submitting Party: Once this missing person is located, you will let Project Jason know as soon as possible. You extend permission to us to include information about the recovery that has been made public, either by law enforcement, the media, or by information you provide. When recovery of the missing person occurs, and you ask Project Jason in writing not to make any of the latter information public, we will respect your wishes. If you wish the case information to be removed from our website, you will let us know.

    • Completed Case Submission: After you submit this form, if you were not able to supply image URLs, you will need to email us photos of the missing person. Multiple photos are preferred, as a person may not look to others as they do in a single photograph. Please use proper case in the form for faster processing. (Not All Caps)

    • Missing Child Requirements: In a parental abduction case, the child must be listed with the National Center for Missing and Exploited Children and visible on their site before submitting this form. In any missing child case, you must be the custodial parent to submit this form.

  • On all the data fields below, please note that the data you provide should be information that can be made public to protect the integrity of the case and your loved one's dignity. If you are not certain, please consult with the law enforcement agency handling the case. Persons of interest cannot be listed unless law enforcement has released their name(s) to the public.
  • The investigative agency refers to the law enforcement agency handling the case. The phone number listed is the phone number the public would use to call in tips and other information to that law enforcement agency. The case number is a matter of public record and should be a part of the information kept by the family of a missing person. We can only accept cases which are listed as missing with a law enforcement agency in the U.S.
